The Reason That Pocket of Fat Won't Shift
And What Can Help
There is a particular frustration in being generally healthy and active, yet having one specific area that behaves as though it has not noticed. A soft fullness under the chin. A lower abdominal pocket that feels out of proportion. This is not a discipline problem. It is anatomy, and it is worth understanding properly before you decide what, if anything, to do about it.
What You Are Actually Noticing
Fat is not distributed evenly across the body, and that distribution is not entirely within your control. Certain areas, particularly under the chin and across the lower abdomen, contain fat cells that store readily but release reluctantly. They are resistant to energy deficit in a way that other fat depots are not.
When you lose weight, fat cells shrink in size. They do not disappear. So even when your overall composition is good, these pockets can remain because the cells are structurally still there, just somewhat reduced.
This is the distinction that matters: general weight loss and localised fat reduction are different goals. No amount of exercise directly dissolves a specific cluster of fat cells in a defined area.
My Approach at Dr Alluris Aesthetics
I trained in neurosurgery before moving into aesthetic medicine, and that background shapes how I think about every treatment I offer. Precision and anatomy are not secondary considerations for me. They are the starting point.
When someone raises fat dissolving in a consultation, my first question is not whether the treatment will work in general. It is whether it is the right choice for that specific person, in that specific area. Some people are excellent candidates. Others might have more laxity than fat volume, which changes the picture entirely. I will always tell you honestly which situation you are in.
How Deoxycholic Acid Works in Practice
The treatment I use for localised fat reduction involves deoxycholic acid, a molecule that occurs naturally in the body as part of the digestive process. When injected precisely into a fat deposit, it disrupts the membrane of fat cells. The cells break down, and the body clears the debris gradually over the weeks that follow.
What makes this clinically meaningful is that fat cells, once destroyed this way, do not regenerate. The reduction in that treated area is lasting. Future weight changes may affect fat cells elsewhere in the body, but the treated area has a permanently reduced number of cells to expand.
The way I administer it matters as much as the product itself. I map the area before injecting, marking a precise grid across the treatment zone with points spaced approximately one centimetre apart. I also use a fanning technique in parts of the area, which means partially withdrawing the needle and redirecting it to cover a broader zone with fewer punctures while maintaining even distribution throughout the deposit. The combination of grid mapping and fanning is designed to produce a consistent, accurate result rather than uneven reduction.
Most people need 2 to 6 sessions, spaced 4 to 8 weeks apart.
Swelling under the chin can last 1 to 2 weeks. Bruising and firmness settle.
Mainly lax skin, pregnancy, or active skin conditions in the area.
What a Consultation With Me Looks Like
We talk first. I want to understand what you are noticing, how long it has been a concern, and what you are realistically hoping for. I am particularly interested in whether those expectations match what the treatment can deliver, because honest information at this stage is the most useful thing I can give you.
I then assess the area. I look at fat volume, skin quality, and how the area relates to surrounding anatomy. From that, I can tell you clearly whether fat dissolving suits your situation, what a realistic outcome might look like, and how many sessions are likely.
There is no commitment at a consultation. Many people find that understanding the anatomy and the options is useful in itself, regardless of whether they proceed.
- Stubborn fat pockets are often anatomical, not a result of lifestyle choices
- Deoxycholic acid destroys fat cells, which do not regenerate after treatment
- Most people need between two and six sessions, spaced four to eight weeks apart
- A grid and fanning injection technique ensures even, precise distribution
- Skin laxity, not fat volume, can make this treatment unsuitable in some cases
- A consultation is required before any treatment to confirm suitability
