Why this kind of fat does not respond to exercise
This is not a willpower problem. The fat stored around the lower tummy and flanks has a significantly higher concentration of alpha-2 adrenergic receptors compared to other areas of the body. During exercise, adrenaline triggers fat breakdown via beta-2 receptors. But in these specific deposits, the alpha-2 receptors dominate and actively block that breakdown signal. The fat is physiologically protected from lipolysis even when the rest of your body is responding normally to exercise and diet.
In women this effect is more pronounced. Oestrogen upregulates alpha-2 receptor expression in the lower abdominal region and flanks, which is why these areas tend to be the last to shift and the first to return.
Why the fat does not come back
This is the part most people do not know. Fat cells destroyed by deoxycholic acid are gone permanently. Unlike diet or exercise, which shrink fat cells without eliminating them, deoxycholic acid ruptures the cell membrane entirely. The body clears the debris via the lymphatic system over the following weeks, and those cells are not replaced.
The reason is rooted in how fat cells behave after childhood. Fat cell numbers increase significantly during two periods only: childhood development and pregnancy. In adults outside of pregnancy, the body does not generate new fat cells in meaningful numbers. Once a fat cell is destroyed, that specific cell is gone for good. The volume in that area does not return in the same way.
How I treat the tummy
The abdomen is mapped into a grid system. Each session covers a maximum of four grids, each measuring 6.5cm by 6.5cm, across the treatment area. This limit is not arbitrary. The active ingredient in fat dissolvers contains caffeine, and exceeding the maximum dose per session carries cardiac risks including palpitations and arrhythmias. Four grids is the safe ceiling per visit.
I use Pluryal for body fat dissolving treatments. Compared to other formulations, it produces less swelling and tenderness, which matters across a larger treatment area like the abdomen.
Within each grid I use one of two techniques depending on the anatomy:
- Spot injections: precise placement at fixed intervals across the grid
- Cannula fanning: a single entry point with the product distributed in a fan pattern beneath the skin, reducing surface trauma
Swelling after treatment is expected and is actually a sign the treatment is working. The inflammatory response triggered by deoxycholic acid is part of the mechanism by which the body breaks down and clears the treated tissue. It is not a side effect to worry about.Tummy at a glance

How I treat the love handles
The flanks follow the same structured approach: a maximum of four grids per session, the same two techniques, and the same Pluryal formulation for the same reasons. The tummy and love handles cannot be treated in the same session as doing so would exceed the maximum safe dose and put the cardiovascular system under unnecessary risk.
Swelling in the flanks is often more visible than in the abdomen given the anatomy, but it settles in the same window and is part of the process rather than a complication.Love handles at a glance
